Begin with what is changing

A retirement, departure or change in services requires more than a replacement signature. Tell us what is changing, the anticipated timing and the responsibilities currently held by the departing physician. Your existing agreements and the clinical situation determine the next steps.

Keep responsibilities clear during the transition

  • Clinical coverage: identify who remains responsible for care, consultations and urgent concerns during each stage.
  • Records: establish authorized access, custody and transfer arrangements so ongoing care can continue.
  • Protocols and delegation: review the treatment menu, provider roles, orders and practice agreements with the incoming clinician.
  • Communication: determine what staff and patients need to know, and who will provide any required notices.

The Medical Board’s departure guidance emphasizes advance notification, access to records and assistance with continued care. The necessary notices and timing depend on the facts; there is no universal transition date promised here.

A practical transition sequence

  1. Review the current structure and agreements with the responsible clinicians and counsel.
  2. Assess the proposed physician’s qualifications, availability and fit with the service menu.
  3. Document the effective handoff, coverage, records access and clinical responsibilities.
  4. Confirm readiness before affected services proceed under the new arrangement.

Do not assume a new agreement automatically transfers prior orders, patient relationships or clinical responsibility. The incoming clinician and counsel should assess what must be reviewed or renewed.

Keep good-faith exams connected to care

Exams for participating practices remain part of the established clinical relationship and proprietary EMR documentation. A transition discussion includes how evaluations, treatment plans and follow-up will remain available to authorized clinicians. It is not a standalone exam purchase.

What to bring to the first conversation

Share your location, services, provider roles and anticipated transition date. Describe concerns without sending patient records through the inquiry form. We can discuss fit and the information needed for a more detailed review; availability and onboarding timing are assessed individually.
